Insurance
Ditchfield Holistic Physical Therapy does not bill insurance. You can submit your receipts for reimbursement. A typed receipt will be provided on a regular basis, which you can submit attached to your carrier’s claim form.
Ditchfield Holistic Physical Therapy is not part of any HMO, Medicare, no-fault, or worker's compensation plans. If your insurance covers out-of-network Physical Therapy, then you should have no problem.​
Employer-based pre-tax medical reimbursement plans will reimburse you for our services.
Cancelation
Your appointment is important and reserved for you. Unless there is an emergency, please be kind enough to cancel your appointment a minimum of 24 hours in advance, preferably 48 hours. Appointments missed without adequate notice will be billed in full.
